When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

If you live in postcode area 3511 or 3512, you are very welcome to register as a new patient at DomstadDokters. You can register as a new patient by completing the form below. By pressing send, your information will be sent to our administration. After this has been checked, you will receive confirmation of your registration.
Practical Information
All consultations by patients with Dutch insurance are declared directly to the insurer. You can find the rates on the website of the Dutch Healthcare Authority, the Nza.
